FACTOR 1:   Knowledge of community health nursing principles, theories, and practices in order to provide comprehensive nursing services to individuals, families, and communities.

 

I have extensive knowledge of community health nursing principles and practices to improve the health of a community by cultivating a healthy environment.  I actively apply healthcare training and medical theories in the promotion of health, disease prevention and preventative education. I consistently apply detailed knowledge of individual or community needs in the provision of quality patient care to people of all races, religions and socioeconomic statuses.  My strategy in community health involves the cultivation of strong relationships and open communication with the community. I actively function in the community, utilizing knowledge of the people and common danger sources to promote and prevent common illnesses. I utilized in depth knowledge of cultural beliefs, values and practices in determining medical needs and appropriate treatment plans. My background as an American Indian from the Tohono O odham Reservation has given me detailed knowledge in adapting medical care to meet cultural requirements. I am skilled in assessing and prioritizing major health problems and implementing a plan to decrease or eliminate problems that contribute to illness development.

I participated in a Community Health Project during Nursing School; my team was assigned to the rural community of Littletown, AZ. I communicated with residents, identified key informants and discovered all available healthcare options in the community. I collaborated in the collection of data, including health and social services, communication, politics and government, economics, education, safety and transportation, recreation, and physical environment.  I developed an intervention plan based on data obtained. My intervention plan included a food drive and supply drive, as well as survey s of residents.

In my role as RN, I serve as patient educator, healthcare program collaborator and client advocate. I function with a daily patient load of 4-6 patients, to whom I serve as advocate in all medical proceedings. I effectively educate patients and families on upcoming procedures, treatment plans and disease symptoms. I graduated from Nurse Preceptor Training in 2007, and now serve as preceptor to nurses and nursing students, educating them on healthcare principles, theories and practices. In my current nursing position, I am responsible for assessing, diagnosing, planning, and evaluating patient condition to determine appropriate treatment plans. I currently serve as RN in the Pediatric Unit, where I am responsibilities for providing patient and family education and health counseling.  I teach patients and families how to handle ongoing and lifelong illnesses, including training newly diagnosed diabetics in handling each stage of the illness. I cover many topics in the education of a diagnosis, including signs, symptoms, medications, insulin injections, finger sticks, central line operations, asthma inhaler usage, and carbohydrate counting.  Diagnosis can be overwhelming to patients, so I make it a point to demonstrate the ease of treatment and manageability of condition to alleviate unnecessary fears. I follow up with a detailed evaluation of their learning to ensure complete understanding of patient treatment plan. I apply many diverse methods of education, based on patient/family learning needs; methods include verbal lectures, handouts, videos, and hands-on demonstrations.  I facilitate education from initial admission to discharge and follow-up.

My current position requires patient education and health counseling on a daily basis. I adapt my instructional methods based on family learning styles and educational levels. I explain risks and dangers of non-compliance with education and health recommendations. I am required to utilize patience and dedication in discovering family learning style and educational level to ensure quality patient care. I also serve as preceptor to both new and experienced RN s and Nursing Students, providing detailed orientation on healthcare policies and procedures. Healthcare staff requires a higher level of instruction that do patients/families, including detailed explanations, medical terminology and source material. Together, my skills in education span high-end education to medical staff to simplified instruction to young patients on how to care for their illness.

FACTOR 3: Ability to meet and deal with a variety of agencies, school personnel, and the community.

               I am skilled in coordinating the services of numerous healthcare agencies and personnel to ensure the highest quality patient care. I collaborate with the Hospitalists, Residents, Interns, Patient Care Technicians, Pharmacists, Respiratory Therapists, Clinical Nurse Leader, and Support Staff in coordinating patient services. When social work issues arise in the course of patient care, I work with Social Worker/Case Managers, Child Protective Services and Police Department. My personal background has trained me to work with a variety of different people to achieve positive results. I grew up on the Tohono O  odham Reservation, and am familiar with the schools, agencies and other resources that are available on the Tohono O  odham Reservation. I also understand some of the language and have experience with tribal interactions. 

               As an RN, I serve as Advocate on behalf of the patient. I not only ensure quality hands-on care, but keep up-to-date on ever-changing state of health. I utilize direct and clear communication skills to ensure my concerns for the patient are heard and addressed. There have been occasions where overworked Interns and Residents overlooked the worsening symptoms of my patient; I addressed the issue, going to superiors when necessary, to ensure quality patient care. I have experience identifying and handling child abuse cases. An infant was brought in with obvious signs of physical and sexual abuse. I notified social services and police as required by law, while also overseeing the care of the patient and keeping the parents away from the infant. I have extensive experience dealing with irate parents, angry families, domestic violence between parents, and disputes between staff members.  I am skilled at handling upset parents whose child were taken away from them and placed in Child Protective Service custody.  Irate and abusive parents called frequently to harass the staff for removing their children from violent and dangerous situations. I collaborate with various medical departments and physicians, ensuring patient safety and protect during transfers to and from the unit.
