Currently I am a Unit Facilitator in a busy V. A. Medical Center Emergency Department, where I am highly successful in first, working with patients to understand and assess their needs and then, coordinating with and supporting our staff in providing both non-urgent and critical medical care.  As a staff member, I consistently work in a team environment where the ability to effectively partner with others is not only required, but crucial to patient outcomes.  In an Emergency Department setting, my job goes beyond providing for emergency healthcare needs, as I must also effectively work with patients, family members, external medical professionals, and community resources to facilitate any follow-up to their emergency care. 

2)Demonstrates ability to prioritize and coordinate patient care placements.

My nursing career has been focused in the Emergency Department of a V.A. Medical Center.  In this fast-paced, time-sensitive environment, it is essential that I strategically prioritize and coordinate the care of both non-urgent as well as critical patients to ensure appropriate response to their medical needs.  This requires a talent for decisive, informed decision making in what can be not only routine, but  life-threatening medical situations.  I am able to quickly identify patient needs and promptly coordinate their care to most effectively meet those needs while ensuring full compliance with both internal and regulatory standards.

                                                           
3) Ability to collaborate with other health care professionals.

In the field of nursing, it is vital that I actively collaborate with multidisciplinary medical/healthcare professionals and ancillary staff in coordinating and facilitating patient care.  I am proficient in interfacing with physicians specializing in all medical disciplines and am able to knowledgeably communicate and work with them to deliver total quality care.

As a credentialed Nurse with 13 years of professional experience, I am consistently applying sound clinical judgment based on extensive background toward effectively assessing, planning, implementing, and evaluating nursing care, with the goal of optimizing patient outcomes.  Working with an Emergency Department, I have had the opportunity to apply these skills toward a broad range of patient healthcare needs, which I believe gives me a unique skill set that might surpass those of a healthcare professional who has specialized in only one area of medicine  I have further applied sound clinical judgment toward defining, developing and implementing practices and policies to facilitate quality improvements.

In a nursing capacity, I am also responsible for a high volume of patient charting/records and am proficient in developing and maintaining complex documentation

 

 

 

5) Ability to identify, analyze, and resolve patient care problems, which result in improved patient care.

 

As a Unit Facilitator, the scope of my responsibilities goes beyond providing patient care.  I am also responsible for applying my 13 years of nursing experience toward clearly identifying, analyzing, troubleshooting, and resolving patient care problems.  I am adept at defining patient care issues, determining solutions-focused responses and developing and implementing targeted action plans to improve patient care.  I am also actively involved in hospital Quality Improvement initiatives and continually contribute my expertise toward supporting the Department and the Hospital in maximizing levels of care.
